Effective IDEA transition planning requires more than meeting compliance requirements—it requires intentional systems that prepare students with disabilities for meaningful life after high school. This session explores a coordinated approach to transition that emphasizes early, purposeful planning across grade levels, strong infrastructure, and sustained interagency collaboration. Participants examines how aligning school‑based transition planning with career development and postsecondary readiness efforts can improve outcomes for students as they move toward employment, education, and independent living. The presentation highlights practical strategies for building and strengthening transition infrastructure, fostering collaboration with partner agencies, and ensuring students have access to tools and supports that promote informed decision‑making and future readiness. Emphasis is placed on how coordinated systems can support districts in moving from compliance‑driven practices to outcome‑focused transition planning.
